Description des services:

A specialized service of Ashby Community Support Services that focuses on individuals with co-occurring ABI and addictions and/or mental issues.
Services include:- comprehensive functional assessment in the home and the community
- intensive case management and individualized community support services geared to the expressed needs and goals of the individual
- 1:1 addiction counselling related to substance use if required
- access to SUBI (substance use and brain injury) group
Essential community outreach services continue in person as needed. Virtual support is an option where appropriate
