Description des services: Foot care services for individuals living with Type 2 Diabetes * early intervention, regular monitoring and treatment of foot related concerns

Service include:
  • foot care education
  • foot risk assessment
  • basic foot care treatment including nail care, corns and calluses reduction
  • consultation and referrals to chiropodists in high risk situations



Frais: Free
Procédé et formulaires: Referral from a DECNET nurse
Admissibilité / population desservie: Must be a client for the diabetes program (DECNET) and referred by a DECNET nurse educator
Langues: English ; French ; Chinese (Cantonese) ; Chinese (Mandarin)
Notes linguistiques: Interpreters available for appointments
