Description des services:
|
Offers long term case management to youth aged 16-24 with a serious mental illness. Service includes individualized, flexible and comprehensive planning. Community Support Workers offer individual and group counselling, support, crisis intervention and linkage to other services and assist in providing a seamless transition to adult mental health services.

Procédé et formulaires:

Referrals are accepted from hospitals, psychiatrists, community agencies, consumers and families.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
