Personnes-ressources publiques: Intake Worker
Description des services: Case management services are offered to individuals living in the Home and Community Care Support Services Toronto Central Region. This recovery-based service includes assistance with finding and maintaining housing, life skills training, accessing community resources and building support networks. Staff liaises with other supports and work collaboratively with them.

Procédé et formulaires: Referrals are made through The Access Point. Referrals can be made by family members, physicians, hospital professionals, mental health and community organizations. Self-referrals are encouraged. To make referral through The Access Point, call or complete the online application form.
